Lavish Beauty RN LLC ("Lavish Beauty RN," "we," "us," or "our") operates a medical spa in Tempe, Arizona. This policy explains what personal information we collect, how we use it, and the choices you have.
Information we collect
We collect information you give us directly, which may include:
- Your name, mobile phone number, email address, and date of birth
- Appointment history and the treatments you receive
- Health information you provide on our patient intake forms
- Payment information, processed by our payment provider and not stored by us
- Messages you send us by text, email, or through our website
We collect this information when you complete patient intake, book an appointment, contact us, or visit the practice. We do not purchase, rent, or otherwise obtain contact information from third-party list providers.
How we use your information
- To provide and follow up on the treatments you request
- To schedule, confirm, and remind you about appointments
- To answer questions you send us
- To send occasional offers and practice news, where you have consented
- To keep the records that Arizona law requires a medical spa to keep
Text messaging
Our intake paperwork asks for text messaging consent in two separate, independent choices, and you may agree to one, both, or neither:
- Appointment and service messages. Replies to questions you text us, appointment scheduling and follow-up, and a request for a review after your visit.
- Offers and promotions. Marketing messages, including special offers and a discount during your birthday month.
Declining marketing messages does not affect appointment and service messages. You receive only the categories you agreed to.
We do not share your mobile number. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All other categories of information exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
Message frequency. Message frequency varies. Most patients receive no more than a few messages per month, and conversational replies are sent only in response to messages you send us.
Message and data rates may apply. Your mobile carrier's standard message and data rates apply to messages you send and receive.
Opting out. Reply STOP to any message to stop receiving texts from us. Reply HELP for help, or call us at 480-933-2328. Consent to receive text messages is not a condition of receiving any treatment or service.

How we share information
We share personal information only in these situations, and never with data brokers:
- With service providers who help us operate the practice, such as our practice management system, scheduling software, and messaging provider. They may use your information only to provide services to us.
- With other healthcare providers involved in your care, where you have asked us to or where the law permits it.
- When the law requires it, or to protect the safety of a person.
- In connection with a sale or transfer of the business, in which case your information stays subject to this policy.
As stated above, your mobile number and your text messaging consent are never shared for marketing or promotional purposes under any of these circumstances.
How we protect information
We limit access to your information to staff who need it to do their work, and we use reasonable administrative and technical safeguards to protect it. No system is perfectly secure, so we cannot guarantee absolute security.
How long we keep information
We keep treatment records for as long as Arizona law requires. We keep marketing contact information until you ask us to remove it or you opt out.
Your choices
- Reply STOP to any text message to stop receiving texts.
- Use the unsubscribe link in any marketing email.
- Ask us to correct or delete your contact information by calling 480-933-2328. Some treatment records must be retained by law and cannot be deleted on request.
Children
Our services are intended for adults. We do not knowingly collect information from anyone under 18 without the involvement of a parent or guardian.
Changes to this policy
We may update this policy from time to time. The date at the top of this page shows when it was last revised.
